Can someone supply me with the correct rules to host a Steam Dedicated Server behind a NAT?

Here is a quote explaining which ports need to be opened/forwarded:

Also, does anyone from Kerio monitor these forums? Feedback is extremely slow, possibly even non-existant!

Feedback might be slow since these discussion forums are not intended to provide technical support.

You need to set traffic rule for each port range you want to allow. The rule must be set to translate destination IP address to IP address of computer in local network (Half-Life server in this case).

For example:

Source: Internet interface
Destination: Firewall
Service: UDP 27010-27012
Translation: translate to address 192.168.1.5 (for example)

etc.
