If you update your mailserver be prepared that normal users (those who are not local administrators) cannot update their KOC. I assumed from reading the documentation that they could (there is absolutely no warning). I got the MSI from the temp folder when I run the KOC setup and run that MSI file from a startup script. That way it can install quietly.

Our users are all restricted users, and we don't have any problems with the auto-update running. It must be something with your local settings if it won't run because of permissions.

Uninstalled KOC, installed old one, run Outlook. Indeed it works in this test. Maybe because some users had Outlook open during the update that things went wrong.

Could fix that by writing people up for not logging off when they leave. I do if they keep ignoring warnings/reminders to log off. Aside from the security implications, it interferes with too many of the scheduled routines that run at night and hampers upgrades.
