Currently we are being blacklisted by AOL, and on the AOL feedback loop. I was able to see yesterday that we are in fact sending spam to AOL. There's only 20 users on our box and none of them are sending. Here in the log is what shows up that corresponds with the aol feedback:

[25/Sep/2007 09:53:07] Recv: Queue-ID: 46f920d2-00000829, Service: SMTP, From: <saxnvvisof<_a.t_>nvvi.net>, To: <d***<_a.t_>worship.net>, Size: 5035, Sender-Host: 87.21.170.211

This is what I see in the headers AOL sent back to us:

Now, we only allow authenticated SMTP. Up until yesterday, we didn't have the SMTP delay, we now have it on 10 seconds. How in the world are these people getting through? Our server is mail.worship.net

Any tips or tricks out there?

Fixed!

Alright, I figured it out. Here is the situation.

Spam was coming into an account on the local server, and not being filtered out by our various filters.

Was then being passed to the correct account, within that account it was setup to forward to another email address. The address in question was an AOL address. That user was marking it as spam from his AOL account.

AOL in turn would look at the last server it came from instead of the originating server, and blacklist our IP address. That is rather unfortunate.

So we get rid of the forwarding that goes to an AOL account wait two weeks, and re-apply for a whitelisting from AOL.
