I'm having inconsistent troubles with my mail server. Mail from (say) googlemail gets delivered with a delay but it gets delivered. Mail from hotmail doesn't come in at all. There seems to be no pattern. Any ideas?

Are you seeing SMTP traffic at your firewall for mail that isn't delivered or delayed? Is the mail bouncing from hotmail? If so, what is the error they're getting back? Are you seeing failed messages or errors during delivery attempts in the SMTP debug logs?

It might not like the name KMS is using. Check the Configuration -> Domains screen for 'Internet hostname'. It should be a FQDN, and should optimally match your PTR record (or your PTR should match the specified hostname). If it can't match exactly, it should at least have the same domain as the PTR record.

Also, make sure you have SMTP debugging enabled in the debug log. Go to Logs -> debug, right-click and choose messages. Enable SMTP Server, and you should be able to see what response you are getting when the SMTP servers are connecting. That might give you a better idea why delivery is failing.
