Check your 'SMTP Server' settings. Make sure that you don't have 'Open Relay' turned on. Best to use 'Users authenticated through SMTP for outgoing mail'.

If you are already set-up that way, then you'll want to see if one of the users accounts has been hijacked and is being used to send spam through their account.

  1. Stop the mail server
  2. Delete the queue directory
  3. Check your relay options, they should only allow authenticated users to relay
  4. Check for weak passwords, someone might have guessed a password and is now abusing the mail account

We had that happen on one of our servers. One client got their password guessed and that was all it took.
