Is it possible to run a GSM gateway with Operator, and if so can you route external calls via prefix?

It should work if you have a SIP-to-GSM gateway. You can configure it as a connection to another PBX in the Operator. You might need to set the rewriting rules since the "Connection to another PBX" does no number rewriting by default.

Regarding prefixes, you can use "specialization" prefixes to do a simple LCR. If your standard dial-out prefix is 9 and you know that mobile numbers start with 7, you can add an outgoing prefix "97" and point it to gateway.
