I have my SPAM filter set to insert **SPAM** into the Subject line but there are random problems. Sometimes an email will have the **SPAM** identifier in Subject but it is not going to Junk Mail folder.

I guess these messages match some mail filters and they are moved to another folders. Check mail filters on server (visible in Webmail) and local filters in mail clients if enabled. (Apple Mail, Outlook, Entourage, Thunderbird...)
You should also enable "Sieve Filters" module in debug log. (WebAdmin -> right click in debug log area -> Messages... -> Sieve Filters)

With external spam filters, my experience is that adding these two lines to the message header will cause the message to be filtered into the Junk E-Mail folder pretty consistently:

X-SPAM-FLAG: Yes
X-SPAM-LEVEL: Medium
