A problem is Operator is perplexing me. Some pins appear blank and so do not prompt for a code when accessing voicemail.

Others are populated, despite never setting this up. I have two boxes and both display this behaviour.

So in short: some extensions have blank PINS and so don't prompt making the function appear 'off'

When I create new user accounts, a pin is automatically assigned.

How do I create blank voicemail pins and thus switch this function 'off'?'

Thanks in advance.

New users are always created with a PIN. It is possible that in some older versions of Operator, the PIN was by default empty.

If you want to erase a PIN, just type something in the PIN text field, erase it and close the dialog. This should update the user with an empty PIN.
