What I have to do? Kerio Operator:2.2.5 build 2976
Sometimes:

Your SIP provider probably doesn't support T.38.

Does it happen only with certain devices, or is it more or less random?

I can analyze some failed transmissions. Usually they are caused by a low quality analogue line, high network latency, etc. There is actually nothing to tweak or configure in Operator.

Could you please gather some debug information:
1. Enable debug logs: Asterisk, Asterisk(detailed)
2. Start packet sniffer
3. Wait until the issue occurs. It would be best, if the dump contained a successful fax and a failed one.
4. Create a support info file (link at the bottom of the system health screen)
5. Email the packet sniffer, debug log and the support info file to fjenicek<_at_>kerio.com. If the files are too large, I can provide you with an FTP address.
