What happens when you switch to delivering outgoing emails using MX record lookups rather than the Exchange online protection? Will that service allow outside servers to use their anti-spam protection?

I think it's worth a try. Also, does Microsoft's anti-spam system require that your users be setup to be allowed to send through their service?

And perhaps you should contact Microsoft for help too. They would have access to their logs to help determine the cause of the issue. If this was my server, I would want an anti-spam solution that I have more control over. I would want to be able to login myself and view logs so that I can take care of issues immediately rather than wait on someone else to reply.

i try to switch to MX record lookup to send email and got the same issue. when i try to telnet from the office it doesnt show the STARTTLS after the command EHLO. but when im the house i can send email using the affected user as mail from. all affected users has a forward option to GMAIL. is it possible that gmail treat the user as spam?
