Kerio is limiting the speed by every conection, but if some body is using download managers which divide the download in multiple conections kerios is ineficient. For example if i have 128Kbyte(1Mbit) speed and i limit to 15KByte, if somebody downloads with a download manager wich divide the download in 10 parts and for every part use a conection makes the bandwith balancer inefective.
So the kerio must limit not by conection but by a summ of conections of one user on this protocol(like HTTP), or generaly by the summ of all connections of the user.

I would assume that the total ammount of bandwith set asside for connections considered 'heavy' is shared by those connections. So if you've sepcified heavy connections to only use 90% of the bandwith, than all heavy connection together do not use more than 90%. This would then apply to all parts of the segmented download.

If the above is untrue, the bandwith limiter would only handle one heavy connection and a second one would render the effects useless. I can't imagine this being the case...

That's correct. And you can set what is considered 'heavy' under the Advanced settings. It defaults to 200KB in 5 seconds.
